16-25 Mental Health Transitions
Transitions and changes in your life can be daunting and difficult to navigate. Our Mental Health Transitions team are specially trained to offer mental health support to young people aged 16-25 in Hampshire around the transitions that happen in their lives.
Our support will be tailored to your individual needs and can be around any transitions you’re going through, including going from child to adult services, and getting support for the first time as an adult.
Tailored support could include

One-to-one support
Direct and individual support from one of our trained youth workers over the phone.

Signposting
Sending you to an organisation local to your area or that can offer more specific support.

Referrals
We can support you to refer into other services that can help your situation.

Support to access other services
Our youth workers can support you to make first contact with, and attend first meetings with, services that can support you.

Advice and support for adults
Help for parents, carers and professionals looking after a young person, including signposting to support and resources.
How do I get support?
To get support from our 16-25 Mental Health Transitions service, fill in a referral form through the link below. If you’re a young person, you can refer yourself. If you’re a family member or professional, you can refer a young person using our Refer a Young Person form.
